PCI Home Care Group, Inc. develops an individualized Care Plan tailored to the unique needs & concerns of each patient. This is done by a Registered Professional Nurse (RN) through an initial clinical assessment of the client as well as the client’s living environment. This serves to capture any issues & what problems there may be that will need to be addressed before a patient is admitted to home care services. This is to ensure the patient’s safety as well as the company staff caring for the client. The RN collaborates with the clients’ Primary Care Physician (PCP) to have a signed Doctor’s Orders for Care before starting the care of client at home. An Aide’s Plan of Care is also developed by the RN to include what the client’s care by the aide ( HHA or PCW) should include as well as provide a supervisory & instruction tool for the RN to use, as appropriate, each home visit. We have Private Duty Nursing Program for medically Fragile Children. Also we have Nursing Registry available to meet your nursing needs.
